Research Project on Mortality Among People Experiencing Homelessness in Québec

About This Research

This form aims to document deaths among people experiencing homelessness in order to improve knowledge about homelessness-related mortality in Quebec.

The information collected is used exclusively for research purposes and contributes to a better understanding of the circumstances surrounding these deaths, as well as their impacts on relatives, communities, and services.

For practical reasons, deaths occurring after June 1, 2026, are included in this study.

Research Consent

✅ Your participation in this research is entirely voluntary. 

By completing this form, you consent to the use of the information you provide for research purposes as part of a project that has received:

  • Ethics approval from the Université de Sherbrooke (Ref. No. 2026-5256/Leblanc).
  • A Privacy Impact Assessment conducted in accordance with Quebec’s Act respecting the protection of personal information (Law 25) and the Act respecting health and social services information (Law 5) (June 10, 2026).

Confidentiality and Data Security

  • ✅ The information provided will remain strictly confidential.
  • ✅ Data collection and management are conducted using secure institutional systems.
  • ✅ Research data will be stored for a maximum period of 20 years.
  • ✅ Access to sensitive data is restricted to authorized members of the research team.
  • ✅ Anonymized data will be shared with the Montreal Regional Public Health Directorate pursuant to a collaboration agreement
  • ✅ Any dissemination of findings will be conducted in a manner that prevents individuals from being identified directly or indirectly.
